Application packets are available at the Merced County Department of Public Health, 260 15th Street, Monday through Friday 8:00 a.m. to 5:00 p.m. or calling (209) 381-1015.
Applications are also available online in English and Spanish.
Completed applications are accepted by appointment only. Appointments are scheduled for Wednesdays in Merced Office from 9:00 am - 4:00 pm. To schedule an appointment please call (209) 381-1015.
At the time of your appointment, you must present:
- A completed MMIC application.
- A valid government - issued photo ID, such as a current and valid mother veheicle driver's license or identification card issued by the California Department of Motor Vehicles (DMV). This includes the DMV DL 43 "Change of Address Certification Card."
- Proof of residency in Merced County such as a current and valid motor vehicle registration, or a current rent or mortgage receipt or utility bill in the patient's name bearing current address within the county.
- Written documentation from their physician recommending the use of medical marijuana as appropriate for one or more serious medical conditions. This documentation must be on physician letter head or on Written Documentation of Patient's Medical Records form (CDPH 9044) which is available online through the California Department of Public Health.
- Your physician's name, contact information and California medical license number.
- Non refundable application fee.
- Current Medi-Cal card if you are a Medi-Cal beneficiary.
