Please include your name used as it was at the time the W-2 was originally issued, your social security number, the year of the W-2 that you are requesting, and a self-addressed stamped envelope. Please sign this request.
You can also request a copy by phone. Please call (209) 385-7511 ext 4282 and provide the same information as stated above.
For W-2s from 2018 to current:
Please contact the department from which you terminated employment with the county. Provide them the same information as listed above.
